Worship Busy Bags

If you and your family are interested in receiving a bag or bags could you please fill out the form below?

Each bag will have your child’s name on it with activities tailored to your child’s age. After worship, please return the bag so that I can refill and reuse it for next time. I will have a little punch card type thing in each bag for parents to mark how many time each child uses their bag and after 5 uses your child will receive a small prize in their bag!
